CONDITIONS FOR ADMISSIBILITY TO THE ELEMENTARY/SECONDARY PROGRAMME
APPLICATION FORM 
To fill in the attached application form, please write in capital letters, and answer every question. Please don’t forget to include your full address and postal code, and to answer the questions about your child’s age, grade, grades completed and the name of their school. Remember to sign your application! You will have to fill in an application even if you received financial support from the programme in previous years.
ELIGIBILITY
You must meet all four of the conditions below in order to qualify for this programme:
1. You are an Indian and registered
with the Department of Indian Affairs and Northern Development;
2. You are a member of the Kahnawake Band;
3. You are a resident of the Kahnawake Reserve;
4. Your child is enrolled in elementary or secondary school.
DOCUMENTS REQUIRED 
You must supply the three following documents:
1. Confirmation in writing
that the student does not meet the criteria to receive financial support
from the education centre of the Reserve;
2. Proof of residence of the parent or legal guardian. This will require
a copy of any two of the following: your gas utility bill, telephone bill,
electrical bill or cable television bill.
3. Letter of admission to an elementary or secondary school.
DISTRIBUTION
OF FUNDS
Payments from this programme are divided; part of the funds are sent directly to the student in the name of their parent or legal guardian, while the school fees are sent directly to the educational institution. Please note that the school fees distributed by Avataq are calculated each year by the Department of Education. These fees correspond to the annual budget allocated to all public educational institutions. If the parents or guardians elect to send the child to a private school, they will have to pay the difference between the amount established by the Department of Education and the fee charged by the private school.
DEADLINE
The deadline for applying for financial support from this programme is August 25th.
